KEVIN PEABODY (Name), being duly sworn, says under
oath:
1. I am an elected or appointed officer or employee of the City of
Lansing, holding the Part-time

2. 1 may derive income or benefit, directly or indirectly, from a contract
with the City or from any City action detailed as follows (Charter 5-505.1):

Prior to my employment I started the referenced jobs contained herein which will
require inspection by the Building Safety Office where I am now employed.

3. 1 have a conflict between a personal interest and the public
interest, the nature of which is disclosed below: (Charter 5-505.2; Lansing
Code 290.04 (I)):
At present time I have approximately 76 installations in the Citywith
my equipment. I wou1 still like to service these if possible. I would also
still like to service the general public, after all, I'm still in

numerous faithful customers who call on me regularly to maintain their
heating and cooling needs.
I have no intentions of installing HVAC equipment or anything that requires
a permit under City Ethics guidelines.
4. 1 have a financial interest in a matter proposed to be acted upon
by the City of Lansing as described below (Lansing Code 290.04(1):

RE: Affidavit of Disclosure
This memo is to clarify my Affidavit of Disclosure.
I'm a licensed mechanical contractor working in and around the Lansing area. I've recently accepted a
part-time mechanical inspector position with the City of Lansing, Building Safety Office. I realize there
could be a conflict of interest in holding both of these positions.
It is my intention to ask the Ethics Board to consider letting me service the general public along with my
list of clients I have worked for in the last four(4) years. I am also adding a list of jobs that are in progress
or are starting in the near future for which contracts had been consummated prior to my date of
employment.
I realize that I will not be authorized to perform mechanical work, for which a permit is required, within
the jurisdictional limits of the City other than the jobs which I have disclosed. I will be happy to address
questions relating to my disclosure at an Ethics Board meeting.
