COUNTY O 4& Subscr'be and sworn to before me this (5 Y da of 19 in County, Mich' an. Notary Public/or DEER H Deputy Clerk Notary P&Ic,Ino m Courtiye County, Wo igan ' My Commission Expires: To: The City Of Lansing Board Of Ethics I am writing this Affidavit of Disclosure on behalf of myself and my family. I currently am an employee of the City Of Lansing, and a five year member of the Lansing Police Department. On February 20, 1998, my wife and I were blessed with the arrival of our first children, (Leith IV,Lauren,Logan,Lindsey,Lucas)the first set of Quintuplets born in Mid-Michigan, and the second set ever in the State of Michigan. Our family has become very blessed to have such healthy, thriving children which are rapidly exceeding the living space within our small three bedroom ranch. In late April of this year, my wife and I were approached by The Lansing Home Builders Association President,Mike Goodall, backed by this State's First Lady Michelle Engler, with the gracious offer to assist us in the construction of a newer, larger home. It has taken us nearly six months to find an ideal location, the construction of which will take place at The River Rock Landing Subdivision, in Dimondale. Ground breaking will begin October 15, 1999, at take approximately six months to complete. Mike Goodall, has named this project"The Curtis Quintuplet Dream Home",the hopes of which to attract generous people and businesses(within the Lansing Area) with its construction. The project savings of this home would be significant, (yet unknown at this time), as they too understand the significant financial strain of trying to raise five babies at the same time. I would conclude by stating that during this campaign, I have never represented myself as a employee of the City Of Lansing, and a member of the Lansing Police Department, in order to obtain any benefits, or make promises to any person or persons in exchange for their donations. I have attached a copy the letter which has been sent to Lansing Area Business, and Home Builder Association Members to solicit items for this project. I hope that a favorable recommendation would be granted for my families future. Sincerely, MUM G GREATER LANSING HOME BUILDERS ASSOCIATION 206 W. They currently live in a small three bedroom ranch home in the Grand Ledge area which they are outgrowing very quickly!!! First Lady Michelle Engler brought the housing concerns of this family to our attention. Since then; we have established a wonderful relationship with the quintuplets. We feel the construction of a custom home is the best way to help them toward a secure future. We are very pleased to announce the formation of the Curtis Family Dream Home Committee who has been charged with building a new home for these special children. The home will be built in Dimondale at River Rock Landing. Ground breaking for the new home is scheduled for this October and Bofysil Construction, L.L.C. has graciously agreed to serve as the project manager. First Lady Michelle Engler, the Honorary Chairperson of the Curtis Quintuplet Dream Home Committee, joins us in encouraging all our members to open their hearts to these children by working with our committee on this unique project . How can you help?? We need "everything" involved in building a new home. We need materials, labor and cash. All participants will be recognized in many ways - and those who provide assistance valued at $500 or more will be listed on the site sign during construction of their home, in the Lansing State Journal and other publications. Cash donations payable to the CURTIS QUINTUPLETS DREAM HOME may be mailed to the GLHBA office. If you are willing to donate or discount labor, materials or provide help in any other way, please call either: Brindley Byrd at (517) 371-1228 or Robin Daniels (GLHBA) at (517) 323-3254 6 b y 6 \ j \
