Vince Cantrell (Name), being duly sworn, says under
oath:
1. I am an elected or appointed officer or employee of the City of
Lansing, holding the position of Code Compliance Officer (Title) in the
Planning & Neighborhood Development Department. This position is:

2. 1 may derive income or benefit, directly or indirectly, from a contract
with the City or from any City action detailed as follows (Charter 5-505.1):
I propose to sell (2) TIFF Detectors to the Building Safety Office for a
total of $60.00. The detectors retail at $150.00 to $250.00 each. I no longer
use the equipment for private housing inspections since becoming a Code
e mpl iaiIr effice The Building nl'ng safety e fi:ce has expressed interesLs±nCe
the Mechanical Section of the Department uses the equipment to detect
Carbon Nbnoxide during furnace inspections

SUBJECT: Purchase of TIFF Gas Detectors
Per the discussion between James Smiertka and Antonia Magistro, Vince Cantrell will need
to file an affidavit of disclosure with the Ethics Board. He can file this affidavit through the City
Clerk's office. Once this affidavit is on file then we can complete your memo dated September 10,
1997.
Please have Krista forward Mr. Cantrell's address,phone number and social security number
to establish a vendor number on the SCT software. Once the vendor number has been established,
Krista can enter a requisition into the SCT software to generate a purchase order to complete the
transaction.
